A cystic lesion in the fetal abdomen may be encountered during the ultrasound examination in the first, second or third trimesters. Apart from the fetal stomach and urinary bladder any other cystic lesion needs to be assessed to arrive at the final diagnosis. Only then is appropriate counseling and prognostication possible. For instance, the counseling for an umbilical vein varix or an ovarian cyst is very different from that for a meconium pseudocyst or a persistent cloaca. This lecture describes a logical ultrasound approach to a cystic lesion in the fetal abdomen.
