Monument’s EPIC Chronic Disease Referrals

Having recognized chronic disease management as a healthcare priority, Monument Health actively engaged in leading and promoting Better Choices, Better Health (BCBH) workshops. Tracking registration and attendance to reinforce lifestyle change was difficult. Implementing the referral process into Epic transferred the steps for assessment, registration, and attendance to qualified BCBH staff.

In addition to serving as director of nursing, diabetes education and case management within the Monument Health system, Shelly Roy, DNP, MHA, BSN, is also a BCBH Master Trainer. With broad program exposure and a system-wide disease management perspective, she is well-positioned to effectively integrate referrals on a large scale.
