Module #3 will focus on utilizing the lens of Person-Centered Planning when moving through Step 3 of the Transition Planning Process, Services and Activities. Utilizing the focus of Person-Centered Planning discussed in Steps 1 and 2 of the Transition Planning Process, discussed in Sessions 1 and 2, participants will expand their understanding to develop services and activities that include courses of study, transition activities, and aligned annual IEP goals. The best practice strategies for these areas will be shared, using practical examples that will support participants in understanding how to incorporate these strategies in their professional practices when individualizing transition plans.
