The purpose of this presentation is to raise awareness regarding students with chronic health conditions and how the education system can empower families to advocate for their children during their school years. Many children are diagnosed with a chronic medical condition prior to graduating from high school. This diagnosis may impact their childhood in a variety of ways. Often, families are not aware of additional resources available to their child and do not consider consulting with the school for such guidance. This presentation will highlight educational needs as well as identify ways to better prepare students for their transition into adulthood when they have a chronic health condition and /or disability.
