I am a fan of the CHW model (prior to coming to the Fry Foundation, we utilized the CHW role in a federal Healthy Start program). Are any of the members of the pilot connected to electronic resource referral systems (NowPow, etc.)? If so, how does that impact the work of the CHWs in the pilot?

Pillars Community Health has received a COVID Contact Tracing Support Department of Public Health and is working with 6 partner organizations to disseminate CCDPH information on contact tracing, build awareness and trust in contact tracing, access to testing and other resources through May 31, 2021. JourneyCare is meeting next week with both NOWPOW and Aunt Bertha's to ascertain which platform to use. I am glad to hear that the CMF CHW pilot is using Aunt Bertha's. Coleman Palliative Care Education I believe also uses Aunt Bertha's.
