Transportation was a common barrier mentioned by prescribers when we detailed in Southern Illinois. I believe IL Medicaid provides some coverage for non-emergency medical transportation but unsure of the specific details. Would be curious if there are more resources available.

I'm not speaking about a particularly vulnerable population, but getting into the mindset for our heart failure topic: someone who has probably seen multiple docs (general and specialist), may have been told they have "heart failure" with varying degrees of explanation about what that means; have probably received conflicting education (eg, "don't eat salt" vs "don't worry so much about salt, it's your water intake you need to worry about"; is taking medications for "blood pressure reduction" even though they don't have high blood pressure... sorry for the rambling... just brainstorming.
