I disagree Nazir. There may be fulminant sinusitis complicating vasculitis, also on occasions intracranial or orbital complications. Imaging should be considered selectively.

Agreed. There would need to be some evidence on nasendoscopy for imaging or something suggesting ic/orbital involvement.

The only caveat I'd add to that is that a trephine offers you the option to insert a cannula or two to irrigate and administer decongestant/steroid drops
