I disagree Nazir. There may be fulminant sinusitis complicating vasculitis, also on occasions intracranial or orbital complications. Imaging should be considered selectively.

The only caveat I'd add to that is that a trephine offers you the option to insert a cannula or two to irrigate and administer decongestant/steroid drops
