Bobbin was spot on in her response and wanted to add that the median only changes over time as it is working to established it's new baseline (so 10 data points, or 10 months of AH #s reported). Once it is set again, the improvement median locks in until a manual process happens to essentially say, "a new quality data date has been identified, therefore we need to starting working on a new improvement median". Ultimately, we want to see the improvement medians updated over time and hopefully in a downward trend to match your systems performance.

All of our data comes off of the Coordinated Assessment. These are completed by all providers, not just CE staff. And we use an assessment that we created ourselves. Not VISPDAT
