I highly recommend SurveyMonkey- super easy to set up questions that collect quantitative and qualitative data

I would also recommend generating a QR code for surveys. I’ve found this effective to get people to fill out short survey after a program. But would definitely second having paper copies for those without phones

We have found that the reason for the survey is an essential question to ask. Is it to improve the program? Is it to garner funds? Is it to aid you in convincing a hospital in the value of investing funds? etc They all need different questions,

Creative Forces: NEA Military Healing Arts Network National Resource Center (www.creativeforcesnrc.arts.gov) is publishing resources around the impacts of the Initiative’s Clinical and Community Engagement programming. Our next webinar, Built to Scale: Creative Forces’ Clinical Research Expansion will present four new pilot studies being funded by Creative Forces to assess the impacts of art therapy and music therapy on military and/or veteran populations on March 16 at 3 pm ET and registration will start next week. NRC will be publishing logic models for arts, health and military community engagement programs in late spring/early summer of 2022.
