I made a mistake and can't answer Heather's question in the QA as a result, so let me answer here what kind of documentation you might have related to these expenses. First, you must document that the recipients are people with significant disabilities. This is a declaration by the individual which is documented. It does not need to be documented with a consumer signature in these time when you might be conducting intake over phone or other remote contact. Staff can ask and record their verbal response. Then you need to document their need or goal in the same way you do with other goals. They can be simple -- increase food security for example -- but there should be a goal. The other thing you document is how the expense is COVID related whether it is for your office or for the individual. I suggest this is written in the individual record when there is an individual response, and in your record of the bill when it is staff or office or community response.
