I'm confused on the client disclaimer. How is the disclaimer in the vendor agreement? The client doesn't see the vendor agreement?
There may be an additional doc that pairs with application for LIHWA that includes "certification" language such as: "I attest that the information stated above is true and accurate and understand that the above information, ifmisrepresented, or incomplete, may be grounds for immediate application termination and/or could result inpenalties as specified by law. I also agree to the additional Release of Information to the water provider asnecessary to process payment and verify services provided. In addition, I agree that data from this form (notincluding my personal identifying information) may be used for reporting or program evaluation purposes." OHCS will follow up with this provision pending DOJ final advisement.
