My question is CLIA. We received a CLIA for one of our dual Residential Care Homes and SNF location. However, we are being told for other 3 that are in this same set-up that we need to provide a letter to add unit to SNF CLIA. We understand testing supplies are driven off CLIA #s. In addition, our legal internally are advising given need test to request separate CLIA

The NH Licensure waiver letter specifically addresses some environmental requirements for sample collection area and testing area - where can we get this full list? It is not identified in the He-P 808 or He-P 817 regulations
