Desk review involves the provider filling out an extremely lengthy survey detailing all aspects of the individual's services. No direct interviews-just someone reviewing the survey

One example of a HCBS deficiency we experienced in a 2-bed licensed home involved an inability to provide community inclusion without transporting both of the residents into the community at the same time. Ironically, when we approached OADS for additional staffing for that home they rejected it, and we do not have two vehicles to provide transportation to the separate individuals at the same time. Moreover, if we wanted to use NET transportation to access the community it must have a corresponding NPI+3 number to make it eligible for NET transportation. Hence, unless every potential community venue has an NPI+3 NET transportation cannot be used.

As Dennis is saying, if a finding is not accurate, we do still need to remedy. Our example is that we had findings stating that clients are not offered the choice of a private room. All of our homes are set up so that every client has a private room so this would not apply to our homes.

I would like to point out that KFI also started with center based supports. We are fortunate that our agency transformation started many years ago. And, of course, it continues today. The outcomes are hard work, but from these stories you can tell they are well worth it!

Provider self-assessments (both residential and non-residential) were required for providers/settings to complete to begin the validation process. Followed up by an Individual Experience Assessment is completed with individuals receiving waiver services.
