I can confirm that enrollment as a CHW is different from any enrollment that a person may have done to be a provider of the Diabetes Prevention Program. RI Medicaid does not to my knowledge have a provider type specific to the Diabetes Prevention Program.

That high of issues you need to have a licensed therapist that may be way out of the scope of a CHW, if you have dx of behavior health you need a licensed practitioner for the safety of the CHWs

How does this work for schools, classroom education re: health topic training? There would be more than 4 students in a classroom. Also, could see CHWs being a great resource for school adjustment counselors, school nurses, etc. re: health education, addressing SDOH?

There should also be a CHW supervisor on site to help manage your CHWS this wouldn't be something managed by a Teacher or Social Worker or Resident Supervisor, they need to have a CHW Supervisor who knows the scope of the CHWs would be my recommendation for all the "out of the box" typical arenas
