I am seeking stakeholders to interview for an NIH Innovation Corps study. We seek insights and observations about the unique challenges that toileting care presents to the care experience, from the point of view of the direct care worker, the care recipient and the family caregivers. We also welcome the point of view from providers, nursing homes, home health agencies to learn if the Toilet Transfer, Toilet Hygiene measures are changing behaviors, used in care plans, etc. Please email pgraham@quainc.com and place "NIH I-Corps Interview" in the subject line. With thanks - important to capture as many points of view as possible!

Colorado definition is this: Direct care workers assist older adults and people with disabilities to carry out basic activities of daily living across LTSS settings (i.e., individual homes, assisted living communities, and nursing homes). Direct care workers include personal care aides, home health aides, and certified nursing assistants.

In the field of Developmental Disabilities, please be aware of the on-line training and competencies for Direct Support Professionals (DSPs) at both the College of Direct Support at the U. of Minnesota Institute on Community Integration and the National Alliance of Direct Support Professionals (NADSP), which provide career ladders, and training consistent with the CMS-HCBS "Setting Rules" and Person-Centered Planning for Long Term Supports and Services (not care) rather than the old model of "taking care of people", reinforcing the stigma of dependency and in conflict with the goals of all federal disability policy, including the Olmstead Supreme Court decision, of independence, interdependence, productivity and community inclusion and integration.

Would be so much easier if we all did the same thing for the next 5 years and modify the program after we see results. Problem will be that the care workers will want more than 15/hr minimum wage and that will put folks back on HCBS waitlist for services. Most states cannot match CA budget or come even close
Katherine Kennedy

Terrific work! This is a relevant publication about occupational credentialing of DCWs" Kim, J. (2020). Occupational credentials and job qualities of direct care workers: Implications for labor shortages. Journal of Labor Research. 41(4), 403-420. https://doi.org/10.1007/s12122-020-09312-5

Agree that taking a uniform approach across the country would be quite challenging, but as we discuss in the guide, ARPA funds are at least one means of making these investments in the near term. Also acknowledge the challenge of sustaining these increases, absent permanency of the kind that was proposed by Build Back Better.

01:32:37
If it's in the contract it must be reflected in the reimbursement rates, and it must include a margin to pay for overtime. Agencies are drowning in overtime costs in Michigan because the $2.35 doesn't include an overtime margin.
Heather Vasek
01:33:05
One problem I have seen is that in the effort to bring everyone under the same umbrella and โ€œlift everyone upโ€, the programs that had higher rates are vilified and are told donโ€™t need to get wage increases because other programs are lower, even though there could be justified reasons for acuity, etc. as to why the rates were higher. The calls for raising everyoneโ€™s wages to $15 per hour means the programs that were able to pay $12 per hour donโ€™t get the same % increase as the others and so their workforce shortage actually gets worse.
Marilyn Sword
01:34:51
We have Certified Family Home providers in Idaho most of whom care for family members with disabilities (kind of like Adult Foster Care) who have minimal requirements for certification and training.
