43:40
HI! Moriah Iverson, MS Business Innovation Manager, Independence First Milwaukee, WI

46:13
We are using DocuSign for signatures.

46:14
We are still not allowed to be face to face in CBCIL

46:19
Brian...New York State is struggling with this too. We are unable to go to the nursing homes. We are having limited contact. Our Open Doors and Olmstead Programs are blocked to enter the nursing homes too. We are still struggling with COVID-19 outbreaks.

47:01
When a consumer has no access to a computer/Internet, how are people assisting with online-only intake info, etc?

47:24
Same here in Ohio we have moved to hands off/ virtual transition. we are ordering online and completing meetings via phone and our referral have increased.

47:58
That's awesome!

48:11
I work for a CIL in Wyoming and we are doing almost the same thing with transitions, I have done three in the last two months and am about to do another. I still have referrals coming in.

49:34
I am from the Ann Arbor Center for Independent Living in Michigan and I am doing all of mine remotely as well and it is working well

50:27
I also am struggling with finding housing in our rural communities

50:37
The Challenges we are facing is how to access personal documentations such as state ID's .

50:37
Accessible, affordable housing is a huge issue and need, especially when it comes to transitions

51:14
I have several coming in as folks want to be transitioned out of the facilities. I have built a very good relationship with all of the facilities in 3 counties.

51:19
Is there a poll we feature we could use to see what percentage of centers on the call have a dedicated transition person or team?

51:20
Yes, accessible and affordable housing stock is dire in WI.

52:47
And I am not confident that “project based housing” is really any better. I question whether they’re becoming modern day segregative settings?

53:25
I have found the ombuds (if available) very helpfull

56:29
Brianhollander DCAI has a dedicated NHT

57:47
Does have other responsibilities other then Transitions at your centers?

58:38
I have a question for Molly

01:00:33
Janet Perreault Disability Network West Michigan- This week Amazon and Walmart, are available for online SNAP purchases in all of the 19 states currently using the system, with 18 more states expected to join in the coming months.

01:01:31
I am interested in this DocuSign. How do you get that.

01:01:50
I have a question

01:01:59
Cleveland, we cover Cuyahoga, Lake, and Geauga counties.

01:02:01
What to do when it's just you the director and 1 advocate currently working remotely?

01:02:06
Do folks in other states have project-based complexes for only pwd and older adults?

01:02:13
I have a question for denise

01:02:18
https://www.docsketch.com/online-signature/

01:03:12
Sorry mic un repairable Moliy do consumers need to be in the nursing facility for 90 days like with the MFP program

01:03:32
Roads to Freedom Center for Independent Living has started an evacuation plan which includes a fast track NHT program

01:04:28
I am wondering if partnerships with local HUD offices, or partnerships with City/County Housing offices are beneficial, and if anyone has tips on building relationships with these types of partners?

01:06:00
wonderful please continue to share resources I will make sure to share the chat and downloads with the recording

01:06:12
We have some misinformation going around telling residents that the process make take years for housing.

01:06:52
How many are having to deal with Managed Care Organizations? This seems to be the stumbling block in Kansas

01:07:27
Moriah Iverson I have found being enthusiastically persistant has been successful and stopping in with cookies every couple of months does wonders lol

01:08:43
In my region of IL, We have MCO (Managed Care Organizations). They have worked well with us, the ones we can get into contact with. They are more helpful in the process as they have access to the insurance side

01:12:22
Tanya Johnson, Superior Alliance for Independent Living in the Upper Peninsula of Michigan. I'm the only navigator up here covering 15 counties, over 16,000 square miles :) I've built fantastic relationships with a lot of nursing facilities, our waiver agency, and housing managers. Sometimes just asking what you can do to help smooths the way.

01:12:55
Thank You

01:13:10
Steve, what resources are you talking about specifically, and how are those resources being allocated to you??

01:13:14
Thank is great'

01:14:37
When discussing building relationships with nursing homes, I have completed face to face visits with nursing home on sort of during off season when Home Choice slows down for us.

01:14:40
If you're doing your signature online/virtual, what application/software are you using to get signatures on your documents?

01:15:41
AND, almost 50% of COVID-19 related deaths in Illinois are people who are living in congregated facilities

01:15:51
I have been sending blank forms to my social workers, and they are scanning/emailing me back the signed documents.

01:16:27
Im using Adobe Software - Adobe Acrobat DC

01:16:30
Denise, what funding do you use to buy the alexa and the ipads

01:16:50
How does that work with confidential and HIPA laws. getting the outside help from Face book

01:17:06
For Money Follows the Person in Ohio the individual does have to be in the facility for 90 days.

01:17:37
Aholmes- yes that is typical they were saying if you use care funds and not MFP then it doesn't matter how long

01:17:59
This is Lisa from Augusta Ga and Walton Options for Independent Living. We are scanning or faxing blank documents that need to be signed and having social workers get them signed and sent back

01:18:23
yes, the question of using FB and other social media for support during/after transition, what about protecting consumer identify?

01:18:52
You call out for volunteers, then you get a consent from the individual to allow you to exchange info with the volunteer.

01:18:55
There is also a free iPhone app called Genius Scan that I am using, it allows you to take photos of your documents, and it essentially scans them in black and white, and you can email them to yourself as a PDF. It’s been a lifesaver after working at home for almost 3 months.

01:19:59
thank you Brian Hollander on using the release

01:20:20
Does your ILC allow for the use of social media on behalf of transition efforts?

01:20:39
for the virtual signatures we just type names of consumer, staff, and witness into PDF form

01:23:14
We also have to consider liability since we are working with a vulnerable population.Volunteers are not always a viable option for us because of that.

01:24:13
For virtual signatures I have started using DocuSign and it has worked really well.

01:26:33
@Frank Rivas have you considered professional volunteers? Like OT Students, PT Students, etc who might need field experience or others who usually have to be vetted to do their field practicum anyways

01:26:33
If anyone is willing to offer to submit their contact information, Roads to Freedom Center for Independent Living of NC PA would love to collaborate with everyone.

01:26:34
yes, transition and diversion are core ILC services

01:26:55
Michelle Resource Center for Disability SOlutions in FL, I'm kinda new, can you explain "MFP"?

01:26:59

01:27:06

01:27:11

01:27:30
@mmurphy I am trying to figure out how to do that now :)

Hello everyone

01:33:50
scuevas@rcdsfl.org

01:34:32
Thank you for your feedback about digital signatures. I have another question. Is anyone having trouble with their local Department of Rehab (DHS) in trying to get a service plan started? My individuals are getting denied and at this point in time, DHS is only doing virtual assessments so more of my individuals are getting denied for home services. Also, my local DHS office is deflecting our reintegration services saying that their department will do it but I have yet to see them take a case that requires more than a few conversations. It is frustrating because they are denying my requests for the assessment or taking my cases and questioning our involvement.

thank you all for the wonderful information. I hate to leave before it's over but I have my IL Networking meeting at 11 am I am late for calling into.

01:41:08
The problem with the use of phones is that there are nursing homes who don't provide their residents with individual phones in their rooms and the only phone is located in the common area. So, staff are not allowing residents to even get phone calls

01:41:15
ninfo@leagueofhumandignity.com

01:41:39

How sad is that?

01:42:01

Corporate and foundation partners want to provide direct assistance- phones are one example.

01:47:02
You would think that with all the money nursing homes get, they would at least provide people with their own phones. Access to communication is a basic human right

01:48:01
@marci Roth do you have examples of the corporate and foundation partners that you are talking about that want to help with direct assistance?

01:48:05
I have a consumer who was not allowed to attend a housing meeting to secure housing. I was told the nursing home was still on lock down. If she does not attend her second and final meeting, she will be removed from the waiting list.

01:49:14
Not yet, the initiative is about to roll out, but I don’t want to wait if there are people in need

Kristen Onaga the billing can be quite daunting and it depends on some of the problems. You can contact me later if you like. I think I have a lot of it down. frivas@cfigj.org

01:54:53
How about in situations where the rehab plans to discharge the resident, even though they have no place to go or family? While an organization can assist with costs into an apartment, what if there is nothing available and there is a wait list for the apartment the consumer can afford?

01:56:16

01:59:46
Michelle Zank-- Discharges have to be considered safe. Discharging a person to the street when they have medical needs that cannot be met while homeless is not a considered a safe discharge and the SNF can be held liable.
