Brian...New York State is struggling with this too. We are unable to go to the nursing homes. We are having limited contact. Our Open Doors and Olmstead Programs are blocked to enter the nursing homes too. We are still struggling with COVID-19 outbreaks.
When a consumer has no access to a computer/Internet, how are people assisting with online-only intake info, etc?
Same here in Ohio we have moved to hands off/ virtual transition. we are ordering online and completing meetings via phone and our referral have increased.
I work for a CIL in Wyoming and we are doing almost the same thing with transitions, I have done three in the last two months and am about to do another. I still have referrals coming in.
I am from the Ann Arbor Center for Independent Living in Michigan and I am doing all of mine remotely as well and it is working well
I also am struggling with finding housing in our rural communities
The Challenges we are facing is how to access personal documentations such as state ID's .
Accessible, affordable housing is a huge issue and need, especially when it comes to transitions
I have several coming in as folks want to be transitioned out of the facilities. I have built a very good relationship with all of the facilities in 3 counties.
Is there a poll we feature we could use to see what percentage of centers on the call have a dedicated transition person or team?
Yes, accessible and affordable housing stock is dire in WI.
And I am not confident that “project based housing” is really any better. I question whether they’re becoming modern day segregative settings?

In my region of IL, We have MCO (Managed Care Organizations). They have worked well with us, the ones we can get into contact with. They are more helpful in the process as they have access to the insurance side

I'm the only navigator up here covering 15 counties, over 16,000 square miles :) I've built fantastic relationships with a lot of nursing facilities, our waiver agency, and housing managers. Sometimes just asking what you can do to help smooths the way.

When discussing building relationships with nursing homes, I have completed face to face visits with nursing home on sort of during off season when Home Choice slows down for us.
If you're doing your signature online/virtual, what application/software are you using to get signatures on your documents?

This is Lisa from Augusta Ga and Walton Options for Independent Living. We are scanning or faxing blank documents that need to be signed and having social workers get them signed and sent back
yes, the question of using FB and other social media for support during/after transition, what about protecting consumer identify?
You call out for volunteers, then you get a consent from the individual to allow you to exchange info with the volunteer.
There is also a free iPhone app called Genius Scan that I am using, it allows you to take photos of your documents, and it essentially scans them in black and white, and you can email them to yourself as a PDF. It’s been a lifesaver after working at home for almost 3 months.

Does your ILC allow for the use of social media on behalf of transition efforts?
for the virtual signatures we just type names of consumer, staff, and witness into PDF form
We also have to consider liability since we are working with a vulnerable population.Volunteers are not always a viable option for us because of that.
For virtual signatures I have started using DocuSign and it has worked really well.
@Frank Rivas have you considered professional volunteers? Like OT Students, PT Students, etc who might need field experience or others who usually have to be vetted to do their field practicum anyways

Thank you for your feedback about digital signatures. I have another question. Is anyone having trouble with their local Department of Rehab (DHS) in trying to get a service plan started? My individuals are getting denied and at this point in time, DHS is only doing virtual assessments so more of my individuals are getting denied for home services. Also, my local DHS office is deflecting our reintegration services saying that their department will do it but I have yet to see them take a case that requires more than a few conversations. It is frustrating because they are denying my requests for the assessment or taking my cases and questioning our involvement.

The problem with the use of phones is that there are nursing homes who don't provide their residents with individual phones in their rooms and the only phone is located in the common area. So, staff are not allowing residents to even get phone calls

We have offered to buy prepaid cell phones if we need too. We also have a tech lending program through tech owl that we can lend things such as ipads
How sad is that?
For technology training there is Senior Planet. They will assist nursing homes set up and train individuals with their own technology or with available technology (i.e. personal Smartphones) to use Zoom or other contact methods of video conferencing.

I had a consumer confined to his room due to positive cases on the floor. I had to get the Ombudsman involved to get a cordless phone brought to the consumer in his room.

Corporate and foundation partners want to provide direct assistance- phones are one example.
You would think that with all the money nursing homes get, they would at least provide people with their own phones. Access to communication is a basic human right
@marci Roth do you have examples of the corporate and foundation partners that you are talking about that want to help with direct assistance?
I have a consumer who was not allowed to attend a housing meeting to secure housing. I was told the nursing home was still on lock down. If she does not attend her second and final meeting, she will be removed from the waiting list.
Not yet, the initiative is about to roll out, but I don’t want to wait if there are people in need

How about in situations where the rehab plans to discharge the resident, even though they have no place to go or family? While an organization can assist with costs into an apartment, what if there is nothing available and there is a wait list for the apartment the consumer can afford?

No SNF should be discharging without proper services in place and having somewhere to go is one of those
Michelle Zank-- Discharges have to be considered safe. Discharging a person to the street when they have medical needs that cannot be met while homeless is not a considered a safe discharge and the SNF can be held liable.

How is your CIL managing the Cares Act. Funding? TCIL made Personal Care assistant the first priority. Tri-County Independent Living is using the Care Act money to cover the partial payment for a private pay caregiving to start while we wait to find a County covered personal care assistant through Medi-caid to help.-Lisa Leon, Transition Coordinator
