Beyond X-ing the X-Waiver
Description
At the end of 2022, President Biden signed into law the removal of the X-Waiver requirement to prescribe buprenorphine to treat opioid use disorder (OUD). This is seen as a first step towards increasing access to OUD treatment, however, X-ing the X-Waiver, on its own, may not drive the increases in buprenorphine prescribing where its needed most. Many primary care providers have not received training in OUD treatment and how to engage patients in long-term treatment or connect them to supports needed for recovery. New providers in all healthcare settings will need training, mentorship, and coaching, and continued efforts are needed to reduce stigma, manage privacy, address disparities, and build a community of practice in which it is expected to care for OUD patients.
Many FORE grantees are showing how these challenges can be met. One grantee is leading a national campaign to encourage other emergency department clinicians to treat OUD patients by leveraging social media and behavioral science to encourage clinicians to get the training they need to start to prescribe buprenorphine. Another is offering OUD treatment education to resident physicians, medical students, and physician assistant students. These are just a few examples of next steps needed to truly expand access to OUD treatment for all.

Westley Clark, MD, JD, MPH (Board of Directors @Foundation for Opioid Response Efforts (FORE)) Dr. Clark is the former Director of the Center for Substance Abuse Treatment at SAMHSA. He contributed to the 2016 Surgeon General’s Report on Alcohol, Drugs, and Health as a Section Editor for Treatment. Dr. Clark is a Distinguished Fellow of the American Society of Addiction Medicine and is a member of the National Advisory Council of NIDA. He is ABAM certified in Addiction Medicine, is licensed to practice medicine in California, Maryland, Massachusetts, and Michigan, and is also a member of the Washington, DC Bar. Alister Martin, MD, MPP (ER Physician & Assistant Professor @Massachusetts General Hospital) Dr. Martin is a practicing ER physician and former Chief Resident at Massachusetts General Hospital. He is dually appointed as an Assistant Professor at Harvard Medical School and as Research Faculty at the Harvard Kennedy School. He leverages his background in politics, healthcare policy, and the field of behavioral economics to use the ER as a place to build programs that serve the needs of vulnerable patients that frequent it. He is the founder of Get Waivered, a program that is converting our nation’s ERs into the front door for opioid addiction treatment. Lara Weinstein, MD, MPH, DrPH (Associate Professor @Thomas Jefferson University) Dr. Weinstein is an Associate Professor and Program Director of the Addiction Medicine Fellowship at Thomas Jefferson University. She works with community partners to expand their model of holistic opioid use disorder (OUD) treatment to more people and gather input from patients about what’s important to them, while also offering OUD treatment education to resident physicians, medical students, and physician assistant students.
