GP Prescribing of Cannabinoid Medicines: First Do No Harm

This activity aims to reiterate core knowledge regarding the types of cannabinoids available via TGA approval in Australia and details their correct use, indications, contraindications, precautions and side effects with particular emphasis on the important differences between cannabidiol and THC.
The activity describes a framework for prescribing medicinal cannabis safely and appropriately by outlining medicinal cannabis pharmacology, toxicology, legal and regulatory requirements, current evidence and approaches to monitoring patients using these medications. This activity accentuates a non-maleficence approach to medicinal cannabis in general practice.

Learning Outcomes:

1. Describe the pharmacotherapeutic principles behind the use of medicinal cannabis including administration routes, dosages, cannabis types and complete and relative contraindications;
2. Explain the regulatory framework underlying safe and appropriate use of medicinal cannabis in general practice, including the use of supporting evidence when applying to treat a condition and use of appropriate documentation processes;
3. Outline the role of and considerations in using medicinal cannabis in persons with previous opiate use disorders, recreational cannabis use and patients who wish to continue driving;
4. Summarise potential side effects, long term risks, safety and toxicity profiles for patients regarding medicinal cannabis when considering this therapeutic option;
5. Demonstrate effective systems for prescribing medicinal cannabis safely, including collaboration with the patient’s non-GP specialists, regular review of patient progress and management of permits and documentation.

Dr Danial Schecter
Physician, Co-founder @Cannabinoid Medical Clinic (CMClinic)
Dr. Schecter's interest in cannabinoid medicine led to the founding of Canada's largest referral only clinic specializing in medical cannabis. He is involved in education and outreach within the medical cannabis space including education and research internationally. As a recognised expert he has trained thousands of physicians and pharmacists on how to help decide if medicinal cannabis is appropriate for their patients. Outside of cannabinoid medicine Dr. Schecter holds a fellowship in Hospital Medicine, is an active hospitalist at the Royal Victoria Regional Health Centre in Barrie, Ontario and also provides house calls to at-risk elderly and palliative care patients.

The first 12 years of his career revolved around a small but busy general hospital in Nova Scotia, Canada, where in addition to his GP duties he worked in: emergency medicine, inpatient care, nursing home care, palliative care, and opioid replacement medicine. Since arriving in Australia 3 years ago, he began to focus his GP practice and recently started a new specialty GP clinic in the Northern Rivers, where he now sees mostly patients with mental health, chronic pain, addictions and palliative care issues. Dr Gunn began integrating cannabinoids into his medical practice early in his career, as a means of harm reduction for opioid dependent patients and now devotes a significant part of his clinical work to seeing referred patients for assessment and treatment with cannabinoids for a variety of conditions. He has been active in providing education to GPs in Australia about the medical use of cannabinoids.
