Whether it's thinking about and beginning the discussion around care for serious issues impacting you or your loved one or completing the formal process of choosing a healthcare proxy and sharing your or your loved ones wishes with a healthcare team, conversations around end of life care can be intimidating. Kate DeBartolo, Senior Director at The Conversation Project will be joining the FOP community next month to highlight the resources available through The Conversation Project as they support individuals through this process. Join us to learn more about starting the conversation, topics to include in the discussion as well as an overview of key forms to complete throughout the process.
The majority of resources introduced in this webinar are applicable to the global FOP community but there will be some reference to U.S. specific documents in the presentation as well.
Simultaneous translation** will be provided in the following languages: Arabic, Bengali, Chinese (Simplified), Chinese (Traditional), Dutch, English, French, German, Hindi, Italian, Japanese, Korean, Polish, Portuguese, Russian, Spanish, Tamil and Vietnamese.
