Bleeding from the Upper GI tract is a surgical emergency which needs to be treated on an urgent basis from the moment the patient presents to the hospital. Though the initial management is by the gastroenterologists and patients are admitted under the care of the medical on call team, it is vital that the surgical team is made aware of the patient. This is particularly important if the initial medical therapy has not succeeded.
So how do we make decisions as to which of these patients would not settle with medical management? What are the options available if medical management fails?
