Self-Measured Blood Pressure Monitoring: Lessons from the Field
In light of the COVID-19 pandemic, organizations have had to rely more on telehealth and remote monitoring of blood pressure in order to provide quality care for hypertensive patients.

Two health systems, Alameda Health System and L.A. County Department of Health Services, have quickly begun to test workflows for engaging and supporting patient self-monitoring with early wins and successful practices that other organizations could learn about and benefit from.

Whether your organization is already doing self-measured blood pressure (SMBP) monitoring or trying to make the case for their organization to try SMBP, this webinar will offer a space to learn from others.

This webinar is being hosted by CCI and Kaiser Permanente's cardiovascular disease prevention programs, PHASE (Preventing Heart Attacks and Strokes Everyday) and TC3 (Transforming Cardiovascular Care in Our Communities).
