Overview
Documentation is an essential element of nursing practice to promote communication amongst care team members and provide a medical-legal record of patient care. The increased complexities of delivering patient care and the adoption of the electronic health record (EHR) have resulted in an increased amount of data as well as rules and alerts which has all contributed to documentation and cognitive burden for clinicians. Strategies are needed to reduce documentation and cognitive burden, which are known contributors of burnout in healthcare professionals. This symposium will outline factors that influence documentation and cognitive burden, strategies for reducing burden, and real-world outcomes associated with decreased documentation burden. A panel discussion will center around success stories and lessons learned in leading hospital and health systems that have decreased documentation and cognitive burden and improved satisfaction at work.
