PCS Virtual Mini-Conference

Proactive Community Services is hosting our first Mini-Conference. The conference theme is
"The South Suburban Response to the Social Determinants of Health: Getting to Zero".

The conference will center the conversation on what leaders, community-serving organizations, and residents can do to support Getting to Zero for Health Disparities in the south suburbs. The four-hour conference will have subject matter experts and local serving organizations provide an overview of the challenges facing the south suburbs. There is also time for you to add your voice on Priority Populations: Women and Girls, men who have sex with men/same gender loving men, Housing, Faith-based and the Elderly, Communicable Disease Response, and Social Determinants of Health. The partner organizations will share what they believe will support Getting to Zero in the
south suburbs, which will be submitted to the local and state departments of health.

Your voice is needed to support the statewide HIV Getting to Zero Plan. This conference is for the south suburbs to get to Zero with all Health Disparities and Social Determinants of Health.
