Vian Nguyen, MD, is a board-certified obstetrician-gynecologist, with a wealth of entrepreneurial leadership experience. She serves as Chief of Clinical Services at Legacy Community Health, guiding organizational clinical strategy, managing gaps in care infrastructure, and overseeing clinical technology acquisition and implementation. Dr. Nguyen is uniquely attuned to the operations of academic medical centers, private practice, and community health centers alike. Widely recognized for her clinical and teaching expertise, she is committed to advancing equity and access while maximizing quality and outcomes. Dr. Nguyen belongs to the board of BioMirror and is passionate about partnering with organizations to mitigate health disparities for underserved patient populations.
Thomas von Sternberg, MD, is a double board-certified geriatric medicine physician who has dedicated his career to innovating care delivery for seniors and elders. He serves as Senior Medical Director of Medicare, Dual Eligibles, and Care Management at Health Partners, consults on care coordination design for Minnesota Senior Health Options, and is involved with the Center to Transform Advanced Illness Care. Dr. von Sternberg is credited with the development of programs and approaches to identify and support patients with dementia, in addition to geriatric assessment clinics, and palliative care, home care, and transitional care programs for assisted living and nursing homes. He has been named a Top Doctor across various publications for the past 15 years.
