Attendees will have the opportunity to hear about the new Remote Patient Monitoring (RPM) programs offered by UnitedHealthcare and Blue Cross Blue Shield of RI (for the Medicare Advantage population). RI Primary Care Physician Corporation will share nurse care manager/patient experiences with use of these RPM programs. In addition, attendees will learn about a unique RPM program being piloted at Providence Community Health Center which is using Health Recovery Solutions and funded through their Accountable Entity opportunity.
This presentation will explain the chronic conditions that are being addressed, technology used, outcomes, and how providers can encourage their patients to participate. Benefits of RPM to providers and patients will also be reviewed. The vendor programs featured include:
