Join Windward Life Care Vice President, Susan Valoff, LCSW, and Director of Clinical Services, Kim Leonard, LCSW, as they explain why advance directives are critical for every adult 18 and older. Webinar participants will learn about the Five Wishes form and its uses as a conversation tool and legal healthcare advance directive.
Estate planning attorney Yvonne Amrine of Amrine Law will join the Windward team for a Q&A session, answering participants' questions about how to document your wishes so they are carried out and address other key documents needed to make sure you and your family are protected.
This webinar qualifies as one hour of continuing education for Certified Aging Life Care Managers with the CMC certification.

She holds a master’s degree in social service administration from the University of Chicago. Susan is a Licensed Clinical Social Worker (LCSW), a Certified Advanced Social Work Case Manager (C-ASWCM), and a Certified Dementia Practitioner (CDP). Susan has been with Windward since 2009 and is a partner in the business. Susan is a member of the "Sandwich Generation," caring for both the older and younger generations in the family, giving her additional empathy to the struggles and joys of family caregivers. A frequent public speaker and active community volunteer, Susan is considered one of San Diego’s experts in aging well. Kim Leonard, MSW, LCSW (Director of Clinical Services @Windward Life Care) Ms. Leonard supervises Windward Life Care's team of Aging Life Care Managers, Nurse Care Managers, and Associate Care Managers. For the past 25 years, Kim has dedicated her career to working with older adults and people with disabilities in San Diego. Kim’s medical social work career has spanned a number of healthcare settings, including skilled nursing; acute care; home health; and hospice. Kim completed her Master’s of Social Work at San Diego State University with an emphasis in healthcare and aging. She is a Licensed Clinical Social Worker in the State of California. Yvonne Amrine, Esq. (@Amrine Law) Ms. Amrine graduated cum laude with a Bachelor’s degree in Business Administration from the University of Southern California. She is licensed to practice law in the state of California and is a graduate of the California Western School of Law. Yvonne obtained a concentration in Elder Law focusing her course work on Estate Planning; Community Property; Estate & Gift Tax and Federal Income Taxation. Ms. Amrine is a member of the California Advocates for Nursing Home Reform and the National Academy of Elder Law Attorneys (NAELA), and serves on the board of the Southern California chapter of NAELA. She is accredited by the Veteran’s Administration to represent clients before the VA Regional Office or the Board of Veterans’ Appeals. She currently serves as a board member at large for the San Diego Planned Giving Partnership and volunteers at the SD Legal Aid Society’s Conservatorship Clinic.
