Family SHADE Transition Symposia

Join us to discuss Medical and Educational Transitions for Young People in Delaware. There will be presentations from transition social workers, PICs transition coordinator and a panel with parents and teens discussing their challenges and successes. There will be light snacks and beverages available as well as some giveaways for participants.
